  • We are allowed or required to share your information in other ways – usually in ways that contribute to the public good, such as public health and research. Website and Electronic Communication

When you complete the appointment request form on our website, that information is emailed directly to us. While email is a convenient form of communication, we cannot guarantee its security. If you provide us with your email address or send us e-mail that contains PHI (such as information about appointments, symptoms, or health concerns), by doing so, you imply that we have permission to respond with e-mail containing PHI.

You have the right to:

  • Obtain a copy of your PHI, with limited exceptions
    • You can ask to see or get an electronic or paper copy of PHI in our records. We may deny you access under certain circumstances. Upon your request we will discuss with you the details of the request and denial process for PHI.
  • Correct your PHI
    • You can ask us to correct PHI about you that you think is incorrect or incomplete. We may deny your request. Upon your request we will discuss with you the details of the amendment process.
  • Request confidential communication
    • You can ask us to contact you in a specific way (for example, home or office phone) or to send mail to a different address. We will say “yes” to all reasonable requests.
  • Ask us to limit the information we share
    • You can ask us not to use or share certain PHI for treatment, payment, or our operations. We are not required to agree to your request, and we may say “no” if it would affect your care.
    • If you pay for a service or health care item out-of-pocket in full, you can ask us not to share that information for the purpose of payment or our operations with your health insurer. We will say “yes” unless a law requires us to share that information.
    • If you have a clear preference for how we share your information in certain situations (e.g. sharing information with your family, close friends, etc.), talk to us. Tell us what you want us to do, and we will follow your instructions provided it does not violate our limits of confidentiality or interfere with your care.
  • Get a list of those with whom we have shared your information
    • You have the right to receive an accounting of disclosures of PHI. On your request, we can discuss the details of the accounting process.
  • Receive a paper copy of this privacy notice
    • You can ask for a paper copy of this notice at any time, even if you have agreed to receive the notice electronically. We will provide you with a paper copy promptly.
  • Choose someone to act for you
    • If you have given someone medical power of attorney or if someone is your legal guardian, that person can exercise your rights and make choices about your PHI. We will make sure the person has this authority and can act for you before we take any action.
  • File a complaint if you believe your privacy rights have been violated
    • If you feel we have violated your rights, please let us know immediately. We will make every effort to make it right.
    • You can file a complaint by sending a letter to the U.S. Department of Health and Human Services Office for Civil Rights.
    • We will not retaliate against you for filing a complaint.

Changes to the Terms of this Notice

We reserve the right to change the terms of this notice without prior notification, provided such changes are permitted by applicable law. The new terms of our notice will be effective for all health information that we maintain, including health information we created or received before we made the changes. The new notice will be available upon request, in our office, or if you prefer, via email.

Spa Policy’s

Check-In Processes:

Please make sure to arrive at least 15 minutes or earlier prior to your appointment to ensure full service.
Upon your arrival, you will be greeted by the Skin 105 MedSpa Staff.

Late Arrivals:

In the event of a late arrival, if possible, we will attempt to accommodate all your needs. However, in consideration of other guests’ time and schedule, Skin 105 MedSpa may not be able to extend your appointment. Late arrivals may very well limit the time of your treatment. Treatments are to begin, and end as scheduled and full value of the service will be charged regardless of arrival time. In these situations, we would be more than happy to service you for the remainder of your scheduled appointment.

Cancellation/No Show Policy:

We understand that there are times when you must miss an appointment due to emergencies or obligations for work or family. However, when you do not call to cancel an appointment, you may be preventing another patient form getting a much-needed service. Conversely, the situation may arise where another patient fails to cancel, and we are unable to schedule you for an appointment. If an appointment is not canceled at Skin 105 MedSpa at least 24 hours in advance you will be charged a fifty dollar ($50) fee.

Booking Appointments:

Please note we take our last appointment for service or treatment at 5:00pm.

Spa Financial Policy

As patients contemplate aesthetic medical spa treatments, they frequently need information about the financial aspects of their treatment and the various payment methods available to them. Our staff members are specially trained in the financial options available to our patients, and they are readily available to assist you with these issues in any way that you may require. Because we provide elective cosmetic procedures, the care provided at Skin 105 MedSpa is not covered by any medical insurance programs, and we do not participate in any such plans.

PAYMENT OPTIONS

Payment for all medical spa procedures is due at the time of the treatment. For specially packaged or grouped treatments, payment for the entire package is due at the time of the first scheduled treatment. A credit card is required to reserve an appointment for treatment scheduled in advance. We provide a number of payment options that may be used individually or combined according to your desires:

  •  CASH
  • PayPal
  • MAJOR CREDIT CARDS: VISA, MasterCard, Discover and American Express
  • FINANCING: Our staff can assist you in obtaining a special line of credit through third parties for financing medical procedures if you wish.

NO REFUNDS FOR SERVICES ALREADY PROVIDED

In the event that a treatment, package, or series of treatments has been purchased, begun treatment or not, these services will be considered to have been rendered even though the full series may not have been completed. Should you wish to discontinue your treatment in the midst of a series, credit for the pro- rata share of unused treatments at the discounted package price may be extended and may be used to purchase other treatments or products offered by Skin 105 MedSpa, it may NOT be transferred to another individual to be used in exchange for treatments or products of comparable value to the credit. This is at the discretion of the practice owner as our Skin 105 MedSpa has a NO REFUND POLICY.
